At Mid and South Essex NHS Foundation Trust, we are gradually introducing a new patient consent system, which will allow patients to give their permission for an operation or medical procedure online, using a digital form.
First, the system will be piloted by patients attending our Breast service from November 2024. The Ear, Nose and Throat (ENT) service will follow shortly after. The pilot will take about three months to complete before the system is introduced in other services across our hospitals. All other services will continue to use paper patient consent forms until the pilot is finished.
This page gives useful information about the new digital patient consent system for anyone attending the Breast or ENT service who needs an operation or medical procedure at one of our hospitals. We will update this page as the new system is introduced.
Consent to treatment means a person must give their permission before they receive any type of operation or medical procedure. EIDO Healthcare is the supplier of our new digital consent system. Their patient consent system offers high-quality patient information leaflets and online surgical procedure video content, alongside digital consent forms. This educational information will help patients understand any medical procedure or operation they are about to undergo before they give their digital permission to proceed.
When you meet with the clinician responsible for your care, they will talk you through everything you need to know about your options. They will ask you whether you would like to sign a digital consent form on-screen, whilst you are at your appointment, or if you want the information about your medical procedure or operation and the digital consent form securely sent to you by email or text message, so that you can read the information and sign your digital consent form at home, in your own time.

Using a digital system for patient consent offers patients more convenience and flexibility. Accessing procedure information at home or remotely, gives time to make an informed decision and could reduce any unnecessary anxiety or pressure felt during a hospital appointment. It may also mean fewer hospital visits.
For us, because the patient consent form will be accessible in our systems, it will improve document and version control, and make it easier and quicker to share information with colleagues across our hospitals and services.
Data is entered into the digital consent application by a doctor or qualified healthcare professional. They update the information in the form to meet your individual needs and this is shared with you via email or text message to review later.
If your email address and mobile phone number isn’t on your health record, you will be asked to give it to us when you next attend an appointment or consultation. The team looking after you will then be able to securely send you a link to the medical procedure or operation information, alongside your digital consent form.
If you do not have a device at home, you can give your consent during your appointment or when you are on the ward – just let the clinician responsible for your care know.
EIDO’s digital consent system is free and easy to use from any digital device that can access the internet - a PC, laptop, tablet or smartphone.
Depending on what is recorded on your health record, you will either receive an email or text message asking you to review and then give your consent.
The email will be sent from no-reply@eidodigital.comto your confirmed email address.
The email will include a link to your education article and digital consent form. You will be able to access it by clicking on ‘GET STARTED’. If you have trouble clicking the button, copy and paste the link at the bottom of the email into your web browser.
You will need to add your surname and date of birth to confirm your identity.
A secure text message will be sent to you from ‘EIDO’ to the mobile number recorded on your health record.

There are five key things to look out for to make sure the message you receive is not a scam:
Once you are in the EIDO system, you’ll see your dashboard including all the education articles which you need to read about your medical procedure. You need to read and confirm that you have read these articles before completing your digital consent form - they tell you everything you need to know about your procedure and will help you decide whether it is right for you.
If you don’t understand something or need further clarification, please contact the team responsible for your care. A contact number will be on the latest correspondence you received from the hospital.
Although the education articles appear in English, they are available to view in some other languages. Currently four other languages are available – Welsh, Arabic, Hindi and Marathi. These will be expanded over time.
On the dashboard, you’ll find the language option. Please click on this to select the language that you prefer.
The consent form is at the bottom of your dashboard, after the education articles.
Once you are happy that you understand everything, you can go ahead and complete your digital consent form. You can use your finger or a compatible electronic pencil to draw your signature.
Once you have completed the digital consent form, the information will be made available to the service responsible for your care. They will contact you to let you know the next steps.
You can go back to your email or text message at any time. Click on ‘GETTING STARTED’ to view your dashboard, re-read your education articles and check your consent form.
On the day of your procedure, the clinician responsible for your care will review your digital consent form with you and make sure you are still happy to proceed.
You can withdraw your consent at any time by contacting your clinician. They will make sure that the system is updated to reflect your decision.
People aged 16 or over are entitled to consent to their own treatment and will receive the procedure articles and digital consent form.
If you are a parent, guardian, person with legal responsibility for another, you will be asked to sign on behalf of the person receiving treatment.
If the person with parental responsibility is not confirmed on the health record of the child, the clinician will need to confirm the email address and mobile number of the appropriate person so that they can be contacted to discuss the medical procedure and be sent the relevant information and consent form in the same way.

If the clinician involved in your care feels that you have the capacity to give your consent, you will be provided with the option to consent either during your appointment or remotely, using the digital consent system.
If the clinician feels you do not have capacity to consent and you have not made an advance decision or formally appointed someone to make this decision for you, the decision on whether to proceed with the procedure will be made by the clinician responsible for your care on your behalf and in your best interest.
To make a decision the person's best interests must be considered. The clinician may consult with the patient’s family members or relations to reach a decision that is right for the patient.

Once the consent form is complete, a pdf version of the form is transferred to our Trust’s Electronic Patient Record (EPR) systems and can be viewed by your healthcare team, at the appropriate time.
